Instructions for completing loan application
  1. Complete the application packet as indicated.
  2. Sign and date the application on page 3 as well as the Broker Fee Disclosure, Credit Authorization and Fair Lending Notices.
  3. Also sign and date page 4 (the addendum page) if needed.
Salaried Applicants:
  1. One month's current consecutive paystubs.
  2. W-2's for the most recent two year period.
Self-Employed Applicants: (those who own 25%+ of a business)
  1. 1040's (Federal Tax Return with all schedules) for the most recent two year period.
  2. If applicable 1165's (Federal Partnership Return) or 1120's (Federal Corporate Return) both with all schedules for the most recent two year period.
  3. YTD Profit and Loss Statement.
All Applicants:
  1. Statements for your investment, checking and savings accounts (including IRA's and 401(k)'s) covering the most recent three month period.
  2. Rental or investment property owners need to provide current lease agreements or complete prior two years 1040's with schedule E.
  3. If recently divorced or separated, please provide a copy of the divorce or separation agreement.
  4. Non-US citizens, please provide a front and back side copy of your green card or visa (either H-1 or L-1).
Applicant's Requesting Cash Out:
  1. Please provide a letter of explanation regarding your intended use of the funds (i.e. investments, home improvements, college tuition, etc.)
